  • Completed and signed
  • $35 credit check fee per applicant (made payable to Punia and Marx, Inc.)

2. Letter of Employment:

  • Must be on company letterhead dated within the last 2 months
  • Must indicate applicant's position at company, salary and length of employment

3. CPA letter or the first two pages of last year's individual 1040


4. Most recent bank statement and/or liquid investment accounts

  • For example, bank letters, money market, stocks, bond etc.
  • Name must be on statement                        

5. Clear copy of photo ID


For your convenience, the building is open for viewing 7 days a week from 9-5.
